Bank 2 is the driver's side. I had that code and tried swapping the CPS from the driver's side to the passenger side to see if it would follow...except I swapped the AVCS sensors instead because I misidentified the sensors. Make sure you correctly identify the CPS. Its on top of the head but all the way toward the back. It can be hard to see. If you're standing on the driver's side, looking at the engine it will be to the right of the fuel rail. BTW: my CPS plug was disconnected, reconnecting it therefore solved the problem.
 
Bank 2 is most certain not the driver side. The driver side is bank 1. If you have the code after changing both sensors or have moved the new sensor to the other bank then check the filter screen on the passenger side AVCS line.
I disagree. I threw that code (0345) when I forgot to reconnect my driver's side CPS after the TGV install. Bank 2 is driver's side (in America, anyway).

Edit: I forgot to add that, if you're still having this issue, the signal from the CPS should be a nice box wave form. If the signal is spiky or flat then its possible you either have damage to your camshaft lobes or a damaged wire in the harness to the ECU. I know its a PITA. but I would suggest you pull the manifold and harness and get it on a workbench so you can trace the wires. It would be extremely difficult, almost impossible, to do while it is installed in the car.
